For your first session, print up the forms to bring with you and fill them out completely.  Enough time is booked between sessions, so you do not have to worry about this cutting into your massage time.  I am currently not doing insurance reimbursement, you may see this on the forms.  You may want to look into what is necessary for you to use your HSA card for massage services.

For maternity massage, there is a physician's permission form.  Please have this filled out by your doctor or health care professional before your first session.  This is to make sure that massage is applicable throughout your pregnancy.

Sometimes massage is a great to use in conjunction with other practitioners.  I have worked closely with other massage/bodywork professionals, chiropractors, acupuncturists, and physical therapists to give the best outcome with a multitude of issues.  The referral form below helps me know what that professional would like to have worked on.
